Adult ADHD is often described as a problem with attention. That is incomplete. Many adults can focus intensely when a task is urgent, novel, interesting, or emotionally engaging. The difficulty is regulating attention and effort consistently across the work that life actually requires.
The question is not “Can you focus?” It is “Can you direct and sustain attention when the task is important but not inherently engaging?”
What an evaluation has to sort out
Poor concentration also appears in anxiety, depression, insomnia, chronic stress, substance use, and medical conditions. A useful psychiatric evaluation looks at the developmental pattern, the degree of impairment, and whether another explanation fits better.
For adults diagnosed later, the history matters especially. The absence of a childhood diagnosis is not the same as the absence of childhood symptoms.
Treatment after diagnosis
When medication is appropriate, follow-up should be tied to function rather than refills alone: task initiation, attention, duration of benefit, sleep, appetite, side effects, and whether the treatment meaningfully changes the problems that brought the patient in.
